A Vehicle Repair Authorization is the form a customer signs to authorize a mechanic or auto shop to diagnose, estimate, and/or perform specific repairs on their vehicle. Under most state consumer protection laws, auto repair shops must provide a written estimate and obtain written authorization before beginning work, and may not charge more than a specified percentage (often 10%) above the authorized estimate without additional consent. This form documents the authorized repair scope, the maximum authorized repair cost, whether the shop may perform diagnostic work before a full estimate, and the customer's rights if additional repairs are discovered. The Shop shall attempt to contact Customer at: ______________. If Customer cannot be reached within 2 hours, Shop shall not proceed with work exceeding the authorized amount. Diagnostic authorization: Customer authorizes reasonable diagnostic work as needed to prepare an accurate repair estimate.
Customer acknowledges that: (a) the estimate is an approximation and may change if additional issues are found; (b) the Shop will not charge more than the maximum authorized amount without Customer's prior approval; (c) Customer has the right to receive itemized documentation of all parts and labor; (d) replaced parts will be made available to Customer upon request (unless required by warranty); (e) Customer may decline any recommended repair; and (f) Customer's vehicle will not be held beyond the time necessary to complete authorized repairs.
Customer will be notified when repairs are complete. Vehicle must be picked up within 3 business days of notification. Storage fees may apply thereafter.
By signing below, Customer: (a) authorizes the Shop to perform the repairs described above up to the maximum authorized amount; (b) certifies ownership of the vehicle or authorization to authorize repairs; and (c) acknowledges receipt of this authorization form.
