This Childcare Enrollment Agreement (the "Agreement") is entered into between ______________, located at ______________ (the "Center"), and the parent(s)/guardian(s) listed below (collectively, "Parent(s)") for the enrollment of their child in the Center's programs.

Child is enrolled in the following program: ______________, commencing ______________. Standard hours: ______________. Parent(s) shall adhere to drop-off and pick-up times consistently. The Center reserves the right to adjust program scheduling with 30 days' prior notice.

Tuition is due regardless of the child's attendance, illness, or holidays, unless the Center is closed due to a Center-declared closure. Accounts more than 5 days past due shall incur a $25 late payment fee per week. The Center may suspend enrollment for accounts more than 2 weeks past due.
Late pickup policy: ______________. All late fees must be paid at pickup; the child will not be permitted to return until all outstanding fees are paid. The Center will attempt to contact Parent(s) and then the emergency contact if a child has not been picked up within 30 minutes of closing. If a child is not retrieved within 60 minutes of closing and no contact is made, the Center may contact local authorities for the welfare of the child.
ONLY the Parent(s) and the following individuals are authorized to pick up ______________: ______________ The Center will require photo identification from any person picking up the child who is not personally known to Center staff. The Center will NOT release the child to any person not listed above, regardless of their stated relationship to the child. Allergies and medical conditions: None reported. The Center will maintain this information in the child's file and train applicable staff accordingly. Parent(s) must update the Center in writing of any changes. For any medication to be administered at the Center, Parent(s) must complete a separate Medication Authorization Form. The Center will not administer any medication without a completed authorization form, even for over-the-counter medications.
In the event of a medical emergency involving ______________, the Center is authorized to: (a) call 911 and emergency medical services; (b) administer first aid as required; and (c) consent to emergency medical treatment if Parent(s) cannot be reached in a life-threatening situation. The Center will make every effort to contact Parent(s) immediately in any emergency.
By signing this Agreement, Parent(s) authorize the Center to take photographs and videos of their child for documentation of developmental milestones, classroom activities, and Center programs. Images may be used in the Center's internal records, newsletters, parent communication apps, and (unless Parent(s) object in writing) in the Center's marketing materials, website, and social media. Parent(s) who object to any public use of their child's image must notify the Center in writing within 7 days of signing.
The Center shall provide guidance and positive discipline strategies to encourage appropriate behavior. The Center does not permit corporal punishment. The Center reserves the right to terminate enrollment with 2 weeks' written notice for: (a) non-payment of tuition; (b) consistent late pickup; (c) behaviors that pose a safety risk to other children or staff; or (d) repeated violations of Center policies. In cases involving immediate safety concerns, termination may be immediate.
This Agreement shall be governed by and construed in accordance with the laws of the State of ______________, without regard to its conflict-of-laws principles.
