I am writing as your tenant at the above-referenced property under the lease agreement dated ______________ (the "Lease"). This letter constitutes formal written notice, pursuant to the implied warranty of habitability under the laws of the State of ______________ and any applicable local housing codes, of the following conditions at the Premises that require your immediate attention and repair.

I previously notified you of these conditions as follows: No prior written notice has been provided, but you were made aware of these conditions verbally. Notwithstanding such notice, the conditions remain uncorrected as of the date of this letter.
Under the laws of the State of ______________, landlords are required to maintain rental properties in a habitable condition, including compliance with applicable building and housing codes materially affecting health and safety. The conditions described above render the Premises or portions thereof uninhabitable and constitute a material breach of the Lease and the implied warranty of habitability.
