I, ______________, residing at ______________ (the "Subject"), hereby voluntarily grant the following authorization to ______________, located at ______________ (the "Company").
Subject hereby irrevocably and unconditionally grants to Company and its licensees, successors, assigns, and designees, the perpetual, royalty-free right and license to use, reproduce, modify, display, transmit, publish, and distribute Subject's: (a) name and any professional or personal pseudonym; (b) photograph(s), image(s), and likeness; (c) voice, statements, and other audio recordings; (d) biographical information; and (e) any testimonials, quotes, or endorsements made during or in connection with ______________ (collectively, "Subject Materials").

Company has no obligation to use the Subject Materials and no obligation to produce, distribute, or exploit any particular media project. Company's rights under this authorization are permissive, not mandatory.
Company agrees not to use the Subject Materials in a manner that falsely states or implies that Subject endorses a product, service, or position that Subject has not approved. Company shall not use the Subject Materials in a manner that is materially defamatory, obscene, or illegal.
Subject represents that Subject: (a) is at least 18 years of age (or, if a minor, that the parent/guardian signing below is the legal guardian of the Subject); (b) is not subject to any conflicting agreement; and (c) has the legal right and authority to grant this authorization.
