This Moving Services Agreement (the "Agreement") is entered into as of ______________ by and between ______________, located at ______________ (the "Mover"), and ______________, located at ______________ (the "Customer").

Customer agrees to: (a) ensure access to both origin and destination on the Move Date at the agreed time; (b) ensure all items to be moved are ready for loading; (c) disclose all large, specialty, or hazardous items in advance; (d) not include prohibited items (hazardous materials, firearms, perishables, cash, jewelry, important documents — take these yourself); (e) provide accurate parking and access information; and (f) be present (or have an authorized representative) at both origin and destination.
Mover may charge additional fees for: (a) stairs, elevators, or long carry distances exceeding 75 feet; (b) shuttle service if the moving truck cannot access the premises; (c) waiting time beyond 30 minutes; (d) additional items not included in the original inventory; (e) storage if delivery cannot be completed on the Move Date. Customer will be notified of any additional charges before they are incurred.
Customer must note any visible damage on the Bill of Lading at time of delivery. Claims for concealed damage must be submitted in writing within 9 months of delivery. Mover shall respond to all claims within 120 days. Customer must file suit within 2 years of delivery.
This Agreement shall be governed by and construed in accordance with the laws of the State of ______________, without regard to its conflict-of-laws principles.
