This Music Instruction Contract is entered into between ______________ ("Instructor") and ______________ ("Student/Parent").

Monthly tuition: $0.00, due on the first lesson day of each month regardless of the number of lessons that month.
Student/Parent must give at least 24 hours' advance notice to cancel or reschedule. Late cancellations or no-shows may be charged in full. Instructor may cancel with 24 hours' notice and will reschedule at no charge.
Regular practice between lessons is essential for progress. Instructor will assign practice materials; Student/Parent is responsible for ensuring adequate practice time.
Student/Parent is responsible for providing or having access to an appropriate instrument and any required books or materials, unless otherwise agreed.
Either party may terminate with two (2) weeks' written notice. Tuition already paid for the current month is non-refundable.
This Agreement shall be governed by and construed in accordance with the laws of the State of ______________, without regard to its conflict-of-laws principles.
