Upon execution of this Agreement and payment of the adoption fee, the Organization agrees to transfer ownership of ______________ (the "Pet") to the Adopter. The Adopter accepts ownership of the Pet in its current condition, acknowledging the disclosures above.
Adopter shall pay an adoption fee of $0.00 upon signing this Agreement. The adoption fee is non-refundable and is used by the Organization to fund rescue operations, veterinary care, and animal welfare programs.
Adopter agrees to: (a) provide the Pet with adequate food, clean water, shelter, exercise, and regular veterinary care; (b) keep the Pet's vaccinations current; (c) ensure the Pet is licensed and registered as required by local law; (d) keep the Pet under appropriate supervision and control; (e) never abandon, abuse, neglect, or use the Pet for fighting or any inhumane purpose; (f) keep the Pet as a household companion — not as a chained outdoor dog, guard dog, or breeding animal; and (g) notify the Organization immediately if the Pet is lost, stolen, or seriously injured.

The Organization reserves the right to reclaim the Pet without refund of the adoption fee if, in the Organization's reasonable judgment: (a) the Pet is being abused, neglected, or mistreated; (b) the Adopter has violated any material term of this Agreement; or (c) the Pet is being used for any inhumane purpose. The Adopter agrees to allow the Organization to conduct a post-adoption home check within 60 days of the adoption.
Adopter acknowledges that rescue animals may have unknown behavioral or medical histories. The Organization makes no representations regarding the Pet's future health or behavior. Adopter assumes all risks and expenses of the Pet's ownership from the date of adoption. All veterinary and care expenses incurred after adoption are Adopter's sole responsibility.
This Agreement shall be governed by and construed in accordance with the laws of the State of ______________, without regard to its conflict-of-laws principles.
