This Pet Care Agreement (the "Agreement") is entered into as of ______________ by and between ______________, located at ______________ ("Provider"), and ______________, located at ______________ ("Owner").
Pet(s) in care: ______________. Owner represents that the pet(s) are in good health, are up-to-date on all required vaccinations, are free from fleas, ticks, and known contagious illness, and are not exhibiting any symptoms of illness as of the service start date. Owner further represents that the pet(s) have no history of aggression toward humans or other animals that has not been disclosed to Provider in writing.

Owner must provide proof of the following vaccinations and preventive care prior to drop-off or service commencement: ______________. Provider reserves the right to refuse service to any pet that does not meet these requirements or that appears ill upon arrival. Owner shall remove the pet promptly if it becomes ill during the service period in a manner that poses a risk to other animals in Provider's care.
Rate: $0.00 per day/visit. Total estimated fee: $0.00. Payment in full is due at or before service completion. Long-term boarding fees are due weekly in advance. Owner authorizes Provider to seek emergency veterinary care for the pet(s) in the event of illness, injury, or medical emergency during the service period. Owner's preferred veterinarian and emergency clinic: ______________. Provider is authorized to approve emergency veterinary expenses up to $500.00 without Owner's prior approval. Provider will make every effort to contact Owner before approving any veterinary treatment. All veterinary costs are Owner's financial responsibility, and Owner agrees to reimburse Provider for any emergency veterinary expenses incurred on behalf of the pet within 5 business days of receipt of the invoice.
Provider agrees to exercise reasonable professional care in looking after the pet(s), including: (a) following Owner's feeding, exercise, and medication instructions; (b) keeping the pet(s) in a safe, secure environment; (c) monitoring the pet(s) for signs of illness or distress; (d) notifying Owner promptly of any health concern, injury, or unusual behavior; and (e) ensuring that gates, doors, and enclosures are secured to prevent escape.
Provider's liability for any loss, injury, illness, or death of the pet(s) during the service period shall be limited to the total fees paid for the service in which the loss occurred. Provider shall not be liable for: (a) pre-existing health conditions; (b) illness contracted from other animals during boarding; (c) escape or injury caused by the pet's own behavior or extreme weather events; or (d) veterinary expenses incurred after the pet is returned to Owner's custody. Provider shall not be liable for any indirect, consequential, or punitive damages.
Owner represents and warrants that: (a) Owner is the legal owner or authorized agent of the pet(s); (b) the pet(s) are current on all required vaccinations; (c) the pet(s) have no known history of aggression that has not been disclosed; (d) all information on pet health, behavior, and medications provided to Provider is accurate and complete; and (e) Owner will notify Provider of any change in the pet's health or medication requirements.
This Agreement shall be governed by and construed in accordance with the laws of the State of ______________, without regard to its conflict-of-laws principles.
