Chemical arrangement: ______________. Owner is responsible for maintaining proper water levels by adding fresh water as needed. Company shall not be responsible for damage caused by Owner's failure to maintain adequate water levels.
Monthly service fee: $0.00. Payment is due on the 1st of each month and shall be paid by [check / autopay / credit card]. Accounts more than 15 days past due may result in service suspension.
If Company discovers equipment in need of repair, Company shall notify Owner promptly. For repairs estimated at $200 or less, Owner hereby authorizes Company to proceed with the repair and bill accordingly. Repairs exceeding the authorized threshold require Owner's express approval before work proceeds.
Company uses industry-standard chemicals and application procedures. Company shall not be liable for damage to pool surfaces, equipment, or surrounding structures caused by: (a) pre-existing damage or conditions; (b) equipment malfunction beyond Company's control; (c) Owner's use of chemicals between service visits; or (d) circumstances beyond Company's reasonable control. Company's liability for any single incident is limited to the value of the monthly service fee.
Owner shall ensure Company has safe access to the pool area during scheduled service visits. If Company cannot access the pool on a scheduled service date due to a locked gate, dog in yard, or other access issue, Owner may be charged for a missed service call.
Contract term: ______________. Either party may cancel this Agreement with 30 days' written notice. Company may terminate immediately for non-payment.
