This Temporary Guardianship Agreement (the "Agreement") is entered into as of ______________ by and between the undersigned parent(s) / legal custodian(s) of the minor child named herein, and the designated Temporary Guardian.

The Parent(s) hereby voluntarily appoint ______________ as Temporary Guardian of the minor child ______________ (the "Child") effective from ______________ through ______________ (the "Guardianship Period"). This appointment is made due to the following circumstances: ______________.
During the Guardianship Period, Temporary Guardian shall have the following authority with respect to the Child: ______________. Specifically, Temporary Guardian is authorized to: (a) Provide for the daily care, supervision, and custody of the Child; (d) Make routine day-to-day decisions regarding the Child's diet, activities, transportation, and social interactions; (e) Travel domestically with the Child, with the Child's travel documents provided by the Parent(s). This appointment does NOT terminate or affect the Parent(s)' legal rights with respect to the Child. The Parent(s) retain full parental rights, and this Agreement does not constitute a permanent transfer of custody or guardianship.
Parent(s) shall remain reachable for communication with Temporary Guardian throughout the Guardianship Period. Parent(s) shall provide Temporary Guardian with all emergency contact information and shall respond promptly to any urgent communication from Temporary Guardian. In a life-threatening emergency where Parent(s) cannot be reached, Temporary Guardian is authorized to consent to all emergency medical procedures necessary to protect the Child's health and safety.
Temporary Guardian shall NOT, without the express written consent of both Parent(s): (a) change the Child's legal name; (b) remove the Child from the State of ______________ (except for short domestic travel not exceeding 5 days); (c) consent to any elective surgery, non-emergency psychological treatment, or experimental medical procedure; (d) grant third parties any authority over the Child; or (e) take any action that would affect the Child's immigration status, citizenship, or legal documents.
Temporary Guardian shall: (a) provide the Child with safe, clean, and appropriate housing; (b) ensure the Child attends school regularly and completes schoolwork; (c) provide nutritious meals and adequate clothing; (d) promote the Child's physical and emotional health; (e) protect the Child from all forms of abuse, neglect, and exploitation; and (f) comply with all applicable state laws regarding the care of minor children.
Parent(s) shall provide Temporary Guardian with sufficient funds for the Child's ordinary expenses during the Guardianship Period, or shall otherwise arrange for the Child's financial support. Unless otherwise agreed in writing, Temporary Guardian shall not be personally financially responsible for the Child's extraordinary medical or other expenses.
This Agreement shall automatically expire on ______________ or upon the return of the Parent(s) and resumption of full parental custody, whichever is earlier. The Parent(s) may revoke this Agreement at any time by delivering written notice to Temporary Guardian. Upon termination, Temporary Guardian shall immediately return the Child to the care of the Parent(s) and shall deliver to the Parent(s) all records, medications, documents, and belongings of the Child.
This Agreement shall be governed by and construed in accordance with the laws of the State of ______________, without regard to its conflict-of-laws principles.
